Sign On ID & Access Code Request Form

Do NOT use this request form to apply for a tax account number.

If you have not received your Sign On ID and Access Code to login to the Alabama Department of Revenue (ADOR) Paperless Filing & Payment System, you may use this form to request the information.  Upon the ADOR receiving a properly completed and submitted request form, we will mail your Sign On ID and Access Code letter to the mailing address we currently have on file for your account.  Allow 4 to 6 business days for delivery.  If you need to talk with an ADOR representative, please call 1-800-322-4106.

In order to receive a Sign On ID and Access Code, you must have a valid tax account number with the ADOR; or for taxes where a tax account number is not required, you must have filed a tax return with the ADOR within the last three years.  To register online for a tax account number, click here.
